Becoming a member of the Heartland Optical Buying Group is a quick and easy process.  Simply follow the 3 steps defined below and we will assign an account number as soon as your information is processed.  There is no start up fee and you can begin using your account as soon as you receive your number.

  1. Print the Group Purchasing Agreement.

    2. View and Print Group Purchasing Agreement

  2. Print the Supplier Identification Form to specify the suppliers you want to bill through Heartland.

    3. View and Print Supplier Identification Form

  3. Return both the Group Purchasing Agreement and the Supplier Identification Form to Heartland.  You can begin using your account as soon as you receive your number.
